Confirm an appointment
Due to COVID-19 epidemic, no walk-ins will be accepted, all patients must have scheduled appointment. Due to the high number of “no shows”, all scheduled appointments must be confirmed one day prior to appointment’s date (by latest 12 noon), any unconfirmed appointment will be rescheduled. To confirm an appointment:
- Call our office at (478) 452-3835 (or scan QR code with your phone) If your child is established patient and being seen for sick visit. or
- Submit Patient’s Intake Form If your child is Established patient being seen for health check, or a new patient regardless of the reason for the visit.
Cancel an appointment
We request that you Call our office at (478) 452-3835 (or scan QR Code with your phone) at least one business day before your scheduled appointment’s date (by latest 12 noon) if you are unable to keep your appointment for any reason, doing so, will enable other patients to have an appointment. We and other patients will appreciate your courtesy.
Schedule an appointment
Patient’s Intake Form is needed If you are Established patient (being seen for health check) or a new patient (regardless of the reason for the visit). We ask that you fill out the Patient’s Intake Form before your office visit.
Please use the section below to request an appointment for your child. All requested appointments will be handled by our staff on the same business day. Confirmation email will be sent to you if the requested date & time are available. Our staff will call you with alternative options if requested date and time are unavailable.
